Now let me see if I can help my Belguim buddy understand this american joke. If i understand you correctly, you understand the "beer" part. Rather than taking a chance of offending some of the ladies here, i'll refer to someing i'm sure you're familiar with. I know Belgium is known for it's Belgium Blue cows. Now those female cows have 4 "tits". Since the guy in the joke was a man, not a bull and he was talking to a lady, go figure.
